How do I sign up and make an appointment with a Plume medical provider?
Step 1: Complete Eligibility Form
Visit https://getplume.co/get-started/ and fill out info accordingly. Questions include your state, zip code, the service you are interested in, chosen name, pronoun, email, birthday, your Medicare status, and how you heard about us. You’ll also need your ID, credit card, and a recent blood pressure reading to complete the sign-up process. Step 2: Select Service Package
Select the pre-filled button that corresponds with the service you are interested in. For patients in New York, New Jersey, and Rhode Island, you will have the option to purchase GAHT for $90 a month or a one-time support letter. If you have an eligible insurance plan, you will have the option to select a package with reduced membership rate of $40
Step 3: Add Billing Information & Complete Payment
Proceed to add your billing information and pay for your initial consultation. After your initial appointment, if you are eligible to continue receiving care with Plume, you will be enrolled in our recurring membership.
Step 4: Schedule your appointment
Once you have completed your purchase, follow the scheduling link in your confirmation email. Schedule your appointment.
Step 5: Download Spruce and Complete Medical Intake
After confirming your appointment and paying, you will receive an email to download Spruce, our secure patient messaging application. In Spruce you will need to complete a medical intake form in advance of your appointment.
What if I am not sure hormone therapy or the Plume membership are right for me?
Receiving gender-affirming healthcare is a deeply personal process, and we are here to help! You can sign up now and schedule an appointment with one of our expert medical care providers. Our whole team specializes in gender-affirming care, and many of us are trans ourselves. At your first appointment, we will discuss all of your needs, desires, and concerns. If you decide that hormones aren’t the right fit for you (or not right for you right now), your fee is a one-time charge. If you are not eligible for care, your initial consultation fee can be refunded
